Primary Health Care Consultations - Placentia-Cape Shore-Whitbourne Area
Overview
Primary health care includes services that promote health and wellness, prevent illness, treat health issues or injuries, and diagnose and manage chronic health conditions. It includes interactions with health-care providers such as family doctors, pharmacists, social workers, nurses, occupational therapists, counsellors, community volunteers and others.
As part of its Strategic Plan 2017-2020, Lighting the Way: Navigating Together, Eastern Health is committed to ensuring access to quality primary health care services throughout the eastern region. This questionnaire provides you with an opportunity to partner with us, and put into motion a plan that identifies renewed priorities and goals in the Placentia-Cape Shore-Whitbourne area - a plan aimed at a transformed model of primary health care, and which can be sustained into the future.
Get Involved
In follow-up to Eastern Health’s Placentia-Cape Shore-Whitbourne area community consultations regarding Primary Health Care services (click here to view a copy of the in-person session workbook), held March 19-20, 2019, we are now conducting an online questionnaire to provide an additional opportunity for people to provide input.
The questionnaire is open to everyone, regardless of whether you attended the consultation or not. We encourage you to take this opportunity to provide your input. The questionnaire will close on April 18, 2019 at 4:00 p.m.
Information collected from both the public consultations and the survey results will be compiled into a report and e-mailed to all participants and posted on the Eastern Health website.
